Serious ethical violations in medicine, such as sexual abuse, criminal prescribing of opioids, and unnecessary surgeries, directly harm patients and undermine trust in the profession of medicine. We review the literature on violations in medicine and present an analysis of 280 cases. Nearly all cases involved repeated instances of intentional wrongdoing, by males in nonacademic medical settings, with oversight problems and a selfish motive such as financial gain or sex. More than half of cases involved a wrongdoer with a suspected (...) personality disorder or substance use disorder. Despite clear patterns, no factors provide readily observable red flags, making prevention difficult. Early identification and intervention in cases requires significant policy shifts that prioritize the safety of patients over physician interests in privacy, fair processes, and proportionate disciplinary actions. We explore a series of 10 questions regarding policy, oversight, discipline, and education options. Satisfactory answers to these questions will require input from diverse stakeholders to help society negotiate effective and ethically balanced solutions. (shrink)

We present a one per cent measurement of the cosmic distance scale from the detections of the baryon acoustic oscillations in the clustering of galaxies from the Baryon Oscillation Spectroscopic Survey, which is part of the Sloan Digital Sky Survey III. Our results come from the Data Release 11 sample, containing nearly one million galaxies and covering approximately 8500 square degrees and the redshift range 0.2 < z < 0.7. We also compare these results with those from the publicly released (...) DR9 and DR10 samples. Assuming a concordance Λ cold dark matter cosmological model, the DR11 sample covers a volume of 13 Gpc3 and is the largest region of the Universe ever surveyed at this density. We measure the correlation function and power spectrum, including density-field reconstruction of the BAO feature. The acoustic features are detected at a significance of over 7σ in both the correlation function and power spectrum. Fitting for the position of the acoustic features measures the distance relative to the sound horizon at the drag epoch, rd, which has a value of rd,fid = 149.28 Mpc in our fiducial cosmology. We find DV = at z = 0.32 and DV = at z = 0.57. At 1.0 per cent, this latter measure is the most precise distance constraint ever obtained from a galaxy survey. Separating the clustering along and transverse to the line of sight yields measurements at z = 0.57 of DA = and H =. Our measurements of the distance scale are in good agreement with previous BAO measurements and with the predictions from cosmic microwave background data for a spatially flat CDM model with a cosmological constant. © 2014 The Author Published by Oxford University Press on behalf of the Royal Astronomical Society. Aims: The Research Diagnostic Criteria for Temporomandi¬bular Disorders (RDC/TMD) Axis I diagnostic algorithms were demonstrated to be reliable but below target sensitivity and specificity. Empirical data supported Axis I algorithm revisions that were valid. Axis II instruments were shown to be both reliable and valid. An international consensus workshop was convened to obtain recommendations and finalization of new Axis I diagnostic algorithms and new Axis II instruments. Methods: A comprehensive search of published TMD diagnostic literature was followed by review and (...) consensus via a formal structured process by a panel of experts for revision of the RDC/TMD. Results: The recommended Diagnostic Criteria for TMD (DC/TMD) Axis I protocol includes both a valid screener for pain diagnoses and valid criteria for the most common pain-related TMDs and for one intra-articular disorder. The Axis II protocol retains selected RDC/TMD screening instruments augmented with new instruments to better assess the interactions between pain and psychosocial functioning. A comprehensive classification system is also presented. Conclusion: The recommended evidence-based DC/TMD protocol is appropriate for use in both the clinical and research settings. Simple Axis I and II screening tests augmented by validated Axis I and Axis II instruments allow for identification of simple to complex TMD patients. (shrink)

Pancreatitis is a complex, progressively destructive inflammatory disorder. Alcohol was long thought to be the primary causative agent, but genetic contributions have been of interest since the discovery that rare PRSS1, CFTR and SPINK1 variants were associated with pancreatitis risk. We now report two associations at genome-wide significance identified and replicated at PRSS1-PRSS2 and X-linked CLDN2 through a two-stage genome-wide study. The PRSS1 variant likely affects disease susceptibility by altering expression of the primary trypsinogen gene. The CLDN2 risk allele is (...) associated with atypical localization of claudin-2 in pancreatic acinar cells. The homozygous CLDN2 genotype confers the greatest risk, and its alleles interact with alcohol consumption to amplify risk. Richard Rorty reimagined the traditional liberal private-public divide to face contingency. This chapter explains Rorty’s understanding of it as a crucial component of any just and stable political order in the post-secular West. Rorty’s continued defense of the liberal private-public divide has been criticized from the political right and left. His foundation-neutral model is, however, equipped (or can easily be modified) to answer these objections. And this is good news, because Rorty has shown us that the leading post-secular alternatives (e.g., (...) identity politics) threaten freedom, risk humiliation, and undermine solidarity. (shrink)

Institutional Review Board decisions hinge on the availability and interpretation of information. This is demonstrated by the following well-known historical example. In 2001, 24-year-old Ellen Roche died from respiratory distress and organ failure as a result of her participation in a study at Johns Hopkins Asthma and Allergy Center. The non-therapeutic physiological study, “Mechanisms of Deep Inspiration-Induced Airway Relaxation,” was designed to examine airway hyperresponsiveness in healthy individuals in order to better understand the pathophysiology of asthma. Participants inhaled hexamethonium, a (...) chemical ganglionic blocker that was not FDA-approved for use as specified in the experimental protocol. Risks of inhaling hexamethonium, including lung damage, had been reported in the 1950s and ‘60s. An investigation conducted after Roche's death determined that the study's principal investigator had failed to adequately search the medical literature; the protocol submitted for IRB review cited articles indexed in online literature databases that went back only as far as the 1970s. Practitioners and advocates of community food security (CFS) envision food systems that are decentralized, environmentally-sound over a long time-frame, supportive of collective rather than only individual needs, effective in assuring equitable food access, and created by democratic decision-making. These themes are loosely connected in literature about CFS, with no logical linkages among them. Clear articulation in a theoretical framework is needed for CFS to be effective as a guide for policy and action. CFS theory should delimit the level of analysis (...) (i.e., what are the boundaries of “community”); show how CFS relates to individual, household, and national food security and explain emergent properties, which are important at the community level of analysis; point to the best indicators of CFS or its lack; clarify the determinants of CFS; and clarify the stages of movement toward CFS. This theoretical base would allow researchers to develop valid and reliable measures, and allow practitioners to weigh alternative options to create strategic plans. A theoretical base also would help establish common ground with potential partners by making the connections to anti-hunger work, sustainable agriculture, and community development clear. (shrink)

The attempt to find usable diagrams fornterms of the sort devised by John Venn seems to have originated with Venn himself, who published diagrams for up to five classes (the fifth class, however, was shaped like a doughnut, and contained an area outside itself — like the hole in the doughnut). Venn then suggested that “if we wanted to use a diagram forsixterms (x, y, z, w, v, u) the best plan would probably be to taketwofive-term figures, one for (...) theupart and one for the non-upart of all the other combinations …” Such a method would, as Venn admits, be somewhat confusing to the eye, and thus fail to fulfil one of the major purposes of the diagram; nevertheless, it is a method which could be extended to covernclasses.Venn also suggests (but without illustration) another method for drawing diagrams fornterms: “… the rule of formation would be very simple. We should merely have to begin by drawing any closed figure, and then proceed to draw others subject to the one condition that each intersect once, and once only, all the existing subdivisions produced by those which had gone before.” Then, in a rather surprising footnote, he adds: “It will be found that when we adhere to continuous figures, instead of the discontinuous five-term figure given above, there is a tendency for the resultant outlines thus successively drawn to assume a comb-like shape after the first four or five. Research based on construal level theory (CLT) suggests that thinking about the distant future can prime people to solve problems by insight (i.e., an “aha” moment) while thinking about the near future can prime them to solve problems analytically. In this study, we used a novel method to elucidate the time-course of temporal priming effects on creative problem solving. Specifically, we used growth-curve analysis (GCA) to examine the time-course of priming while participants solved a series of brief verbal problems. Participants (...) were tested in two counterbalanced sessions in a within-subject experimental design; one session featured near-future priming and the other featured far-future priming. Our results suggest high-level construal may temporarily enhance analytical thinking; far-future priming caused transient facilitation of analytical solving while near-future priming induced weaker, transient facilitation of insightful solving. However, this effect is short-lived; priming produced no significant differences in the total number of insights and analytical solutions. Given the fleeting nature of these effects, future studies should consider implementing methodology that allows for aspects of the time-course of priming effects to be examined. A method such as GCA may reveal mild effects that would be otherwise missed using other types of analyses. (shrink)

Neste artigo, nossa tarefa será estudar a estratégia literária usada na composição da narrativa do encontro de Jesus com a mulher samaritana, famosa passagem do quarto capítulo do evangelho de João. Defenderemos a hipótese de que o autor fez, para a construção desta narrativa sobre Jesus, uso de um enredo arquétipo, uma cena-padrão do Antigo Testamento que era usada todas as vezes que se pretendia contar uma história de casamento. Veremos os elementos que constituem tal enredo padrão e alguns exemplos (...) textuais, para então analisar o evangelho de João demonstrando em detalhes como o evangelista manipulou este gênero literário popular para atingir seus próprios objetivos. Metodologicamente falando, é a análise narrativa ou narratologia que conduzirá nossas reflexões e análises exegéticas, mantendo o foco desta investigação sempre na figura do “leitor implícito” da narrativa bíblica, que é quem deveria surpreender-se ao notar que o texto conduzia Jesus ao próprio casamento, até uma reviravolta criativa do autor no final. Palavras-chave : Exegese; Narratologia; Bíblia; Cristianismo Primitivo; Evangelho de João.In this article, our task will be study the literary strategy used in composition of the narrative of meeting among Jesus and the Samaritan woman, famous episode of fourth chapter of John’s Gospel. We’ll defending the hypothesis that the author made, for to construct this narrative about Jesus, use of a master-plot, a standard scene of the Old Testament that was used all times when someone wished to tell a marriage store. We’ll see the compositional elements of this standard plot and some textual examples, and then, we can to analyze the John’s Gospel and show how the evangelist manipulated the popular literary genre for reach your own objectives. Methodologically speaking, the narrative analysis (or narratology) going to drive our reflections and exegetical analysis, keeping the focus of this research always in “implicit reader” of biblical narrative, who would to feel the surprise through to see the text driving Jesus to the own marriage, until the creative author’s turnaround at the end. Skeptical theism (ST) may undercut the key inference in the evidential argument from evil, but it does so at a cost. If ST is true, then we lose our ability to assess the all things considered (ATC) value of natural events and states of affairs. And if we lose that ability, a whole slew of undesirable consequences follow. So goes a common consequential critique of ST. In a recent article, Anderson has argued that this consequential critique is flawed. (...) class='Hi'>Anderson claims that ST only has the consequence that we lack epistemic access to potentially God-justifying reasons for permitting a prima facie “bad” (or “evil”) event. But this is very different from lacking epistemic access to the ATC value of such events. God could have an (unknowable) reason for not intervening to prevent E and yet E could still be (knowably) ATC-bad. Ingenious though it is, this article argues that Anderson’s attempted defence of ST is flawed. This is for two reasons. First, and most importantly, the consequential critique does not rely on the questionable assumption he identifies. Indeed, the argument can be made quite easily by relying purely on Anderson’s distinction between God-justifying reasons for permitting E and the ATC value of E. And second, Anderson’s defence of his position, if correct, would serve to undermine the foundations of ST. (shrink)

We present a cross-sectional study of ethical decision-making correlated with spirituality and utilizing moral intensity as a moderator for workers in the Southeastern United States. This study presents spirituality as an individual variable and moral intensity as a situational variable along with ethical decision-making to examine the interaction of these factors in moral dilemmas. Utilizing previously validated instruments for ethical decision-making and individual spirituality, we find that workers with relatively high measured spirituality made less ethical decisions compared to workers with (...) relatively lower measures of spirituality. Further, we find that the introduction of high moral intensity as a situational variable does not moderate the observed correlation between spirituality and ethical decision-making. This research supports the conceptual nature of the Interactionist Theory by presenting in a single study both individual and situational variables in ethical decision-making. (shrink)

Anderson and Belnap's most explicit characterization of the fallacy of modality is as follows: "Modal fallacies arise when it is claimed that entailments follow from, or are entailed by, contingent propositions." The view which Nelson attributes to Anderson and Belnap, on the other hand, is "that necessary propositions are entailed only by necessary ones, never by contingent ones." Anderson and Belnap speak of "entailments," whereas Nelson generalizes to "necessary propostitions." The move is far from innocent, as (...) we shall see. For though Anderson and Belnap hold that every true entailment is a necessary proposition, not every necessary proposition is an entailment. In order to grasp the implications of this distinction more clearly, let us see how Anderson and Belnap's statement of the modal fallacy might legitimately be restated in terms of necessity. To that end we must draw a related distinction between necessary propositions and necessity propositions. A expresses a necessary proposition iff A → A → A holds. A expresses a necessity proposition iff A is of the form B → C. The particular form of these definitions is dependent on the fact that entailment rather than necessity is primitive in Anderson and Belnap's system; the forms in parentheses are the equivalents in terms of necessity. Now it follows from these definitions that every entailment is a necessity proposition and that every necessity proposition is an entailment. The modal fallacy may thus be equivalently characterized as the claim that a necessity proposition is entailed by a contingent proposition. That this is different from Nelson's formulation is clear from the following considerations. First, not every necessary proposition is a necessity proposition; example: "Either it is raining or it is not raining." Second, not every necessity proposition is a necessary proposition; example: any false necessity proposition. Thus the possibility is left open that some necessary proposition might be entailed by a contingent one. This is exactly the case with Nelson's example 2, "That every polychromatic surface is red entails that every polychromatic surface is colored." This might well be a theorem in a suitable extension of E. Such cases do not, however, arise among the theorems of EI. In that limited system, all that a contingent proposition can entail is itself. The task at hand is to review work on the history of early American pragmatism from the last ten years. However, writing on the history of pragmatism presents us with a different problem than, say, dealing with historical accounts of Mill’s Logic. The meaning of ‘pragmatism’ is routinely contested and, likewise, who is to count as a pragmatist is contested. The issue, of course, arose soon after William James named “pragmatism” in his 1898 talk at Berkeley titled “Philosophical Conceptions and (...) Practical Results.” The discussions of pragmatism that ensued in journals soon thereafter marked James, Charles Peirce, John Dewey, and F. C. S. Schiller as pragmatists. There were, as well, a number of “friends” of pragmatism such as Addison Moore. There were also clear-cut opponents of pragmatism including the likes of Paul Carus and James E. Creighton. But issues quickly muddied the waters. American idealist Josiah Royce, objecting to what he took to be the intellectually loose Jamesian-Schillerian strand of pragmatism, named them ‘pure pragmatists’, and then... (shrink)

These are considerations for natural philosophy; Locke counts himself rather among the moral philosophers. He does not propose, therefore, to meddle with the physical aspects of the mind, (...) nor with the physical production of sensations through the organs, nor with the production of ideas in the understanding, nor even with whether ideas depend upon matter for their source. He defers to the wisdom of such men as Boyle, Sydenham, Huygens and Newton in these considerations, and finds it "... ambition enough to be employed as an under-labourer in clearing the ground a little, and removing some of the rubbish that lies in the way to knowledge....,, 1 It is true that Locke's Essay is characterized chiefly not by an interest in matter and the mechanics of perception but rather by an interest in epistemology and in the ordering and assessment of our knowledge. Because of the very nature and emphasis of this epistemology, however, Locke finds that he must, from time to time, at least mention the nature of matter and how it affects us. Finally, it becomes apparent, certain changes or ambiguities in his point of view regarding the mode of our knowledge and its certainty are consequences of his understanding of matter. It is a demonstration of these elements of his doctrine, which seem not to have obtained the full attention of such scholars as Aaron, Fraser, and Gibson, that I shall attempt in this paper. Fundamentally, Locke's theory of matter appears to be that of Democritus and Epicurus. Like the Greek atomists, he conceives of matter as consisting of particles or corpuscles infinite in number. At least some of these are so minute as to be individually insensible, and probably he intends that all are, although, to my knowledge, he never states that this is so. In the aggregate, in any case, particles do constitute perceptible bodies. As for the Greeks, so also for Locke, the particles arc all extended, having size and shape. They are said also to possess motion or rest, although he takes care to say that matter cannot produce motion in itself,e Locke frequently mentions other properties as being "primary" or "really in" material things.
